TITLE: Nurse Consultant
REPORTS TO: Program Director
HOURS: Full-time Monday – Friday, non-exempt position
STARTING PAY: $40.00 per hour (depending on experience)

JOB SUMMARY: CISC is a non-profit organization, founded in 1972, that supports immigrants and their families by creating
opportunities for them to succeed, while honoring their heritage. We provide youth, family, and senior services including
preschool, after-school, ESL/naturalization, in-home care case management, and healthcare navigation programs. We
serve Chinese, Russian, Latino, Vietnamese and other immigrant communities at our office locations and numerous
community-based sites throughout King County, Washington.
The Nurse Consultant works independently, and closely with the professional case management team, to provide
cultural- and language-appropriate in-home care case management services to disabled adults and elderly of the
Chinese community in Seattle/King County. Frequent home visit is required for this position. This is a Full-time (40 hours
per week) position.
JOB DUTIES:
Provide nursing consultation services to a team of professional case managers for the Seattle/King County in-
home care program which include reviewing case files, conducting psycho-social-mental assessment, providing
health care training to individual care providers and case managers
Support in-home care case management activities by working closely with case managers through an
interdisciplinary approach, in identifying medical and health care needs of the clients and recommend
appropriate service plans for the clients
Monitor the health care and medical needs of the clients through follow-up with physicians, pharmacist and
health care providers
Provide individual health care counseling to clients, family members and individual care providers
Prepare monthly and quarterly reports
Participate in network meeting when required
Other related duties assigned
QUALIFICATIONS:
Current, active Washington state Registered Nurse licensure
Minimum of 2 years nursing experience in a social services or institutional setting
Knowledge and experiences in working with Non-English speaking or Low-English-Proficiency ethnic Chinese
seniors and disabled adults
Fluency in both verbal and written English and Chinese languages (both Cantonese and Mandarin dialects) is a
must
Must be able to use computer and information technology for client record documentation
Valid Washington State driver’s license and auto insurance, and access to vehicle for personal use for outreach
and home visit
